4. Copy of Deed(s) of Trust
   (This includes 1st, 2nd, 3rd, etc. and HELOC loans;
    All Loans against the property)
   

5. Property Tax Bill (most recent)
   

6. JV Agreement
   
   Please VERIFY You have: Initialed ALL disclosures

AFTER completing these forms, you are ready to submit them to your U1st Representative, Mark Antonowsky.
